Flexible Procurement Design Supports Complex Benefit Structures

Two large employers were navigating the challenges of comparing traditional procurement options to a carved-out approach. PSG was able to help standardize the process and simplify the decision-making effort.

The Challenges

One large employer was operating within a highly complex pharmacy benefits environment involving multiple PBMs and vendors across their plans. As part of a strategic transformation initiative, the employer sought to evaluate both traditional full‑service PBM models and a Modular Pharmacy Benefit Administrator (PBA) model, an approach that would allow them to select best‑in‑class partners for individual service components and effectively “build” their own PBM solution.

Another large employer set out to evaluate both traditional full-service PBM models and carve-out options for mail and specialty pharmacy. Their pharmacy benefits were complex, with numerous plan designs, legacy populations, and varied pricing structures like point-of-sale rebates and acquisition-based terms. This complexity made it difficult to compare vendor proposals directly.
