The transition from NCPDP D.0 to F6 is one of the most significant pharmacy transaction standards changes payers have faced in over a decade. The final rule was published in December 2024, with an effective date of April 14, 2025, and full compliance required by April 14, 2028.

An eight-month transition period begins August 14, 2027, during which trading partners may use either the current or updated standards by agreement. Organizations should not treat this transition period as the starting point, since readiness, partner alignment, and end-to-end validation take time.

This change impacts more than point-of-sale claims. The updated standards apply to health care claims or equivalent encounter information, eligibility, referral authorization, coordination of benefits, and a modified Medicaid pharmacy subrogation transaction, with related batch standards also adopted.
